Flying Rabbit Customer Waiver, Release of Liability, Indemnity and Assumption of Risk Agreement 
 
In consideration of being permitted by Glycon Industries LLP, doing business as “FLYING RABBIT”, to participate in its activities and to use its equipment and facilities, now and in the future, I hereby agree to release, indemnify and forever discharge FLYING RABBIT, Glycon Industries LLP, owners, members, shareholders, officers, directors, managers, partners, employees, volunteers, manufacturers, participants, lessors, successors on behalf of myself, my spouse, my child(ren), my parent(s), my heir(s), assign(s), personal representative(s) and estate as follows:
 
1. NATURE OF THE ACTIVITIES.
Glycon Industries run a trampoline and adventure park where visitors may actively engage in or merely observe various trampoline and park-related activities such free jump, professional trampoline, dodgeball, basketball, foam pit jumping, Zumba, wipe out, ninja warrior course, battle beam, climbing wall, sky stepper, crazy jump.
 
2. TYPES AND ASSUMPTION OF RISKS.
I agree that there might be inherent risks associated from engaging in the activities. These risks include, but are not limited to, cuts, bruises, muscular strain, twisted or sprained ankles, knees, shoulders, or wrists, burns, dirt or other objects in the eye, concussions, broken bones, bodily injuries, landing incorrectly, overexertion, failure of the attraction surface or attachments, being struck by a ball, double bouncing (collisions with other participants).  Given the nature of the activities, there are additional risks and hazards beyond those mentioned above, as well as unknowable and unpredictable dangers.
Despite the hazards mentioned above and the safety precautions taken by Glycon Industries, I acknowledge that there is always a chance of harm and is aware of the demands of the activities in relation to my physical condition and skill level. I affirm that participation in the activities is voluntary and I knowingly, with understanding of the risks and potential injuries, assumes all risks inherent with the activities and access to the premises. I accept all responsibility for my own conduct, safety, and welfare and acknowledge the hazards connected with these activities of flying rabbit trampoline and adventure park.
 
3. CONSUMPTION OF TOXIC SUBSTANCES.
I confirm that I have not consumed any alcohol, tobacco, drugs, or participated in any other activity which might damage my ability to safely participate in the activity prior to or during the course of my participation in any of the activities located at flying rabbit’s facility.
 
4. RELEASE AND INDEMNITY.
I on behalf of myself, my child(ren), my spouse, my parents,  executors, representatives release and agree not to sue, and shall indemnify Flying Rabbit Trampoline and Adventure Park, Glycon Industries LLP, its owners, partners, officers, shareholders, members, managers, agents, employees, contractors, representatives, heirs, assigns, volunteers from and against all liability for my (or my child’s) losses, damages, claims, demands, actions of any kind which may hereafter occur to me (or my child) including my traveling to and from this activity, costs, death, accident, disability, personal injury, property damage, property theft, property loss due to my active or passive participation in the activities.
I hereby agree to not carry weapon of any kind, professional photography equipment or cameras with detachable lenses (without permission), illegal and flamable substances, fireworks, pets, sharp objects, outside food in gaming arena which can cause damage and harm to the game setup and premises of the flying rabbit. I am completely liable for the recovery cost if caused any.
 
I confirm that I have not felt any flu-like or common cold symptoms in past 14 days and don’t have any medical condition related to heart and respiratory system. I also understand that I will have to follow strict guidelines to help prevent each other from the spread of contagious covid-19 virus
 
I have read and understood all the above terms and conditions. I agree to be bound by these terms and conditions on behalf of myself, my child(ren), my spouse, my parent(s), executor(s), personal representative(s).
